CLINICAL SOCIAL WORKER – PEDIATRIC & FAMILY SUPPORT SERVICES

BloomKidz is seeking a full-time Licensed Social Worker (LMSW) in Chandler, AZ. This role is designed for a clinician who is passionate about supporting both children and families through an integrated model of therapeutic care, family advocacy, and case management support.

POSITION SUMMARY

The Clinical Social Worker provides direct therapeutic services to pediatric clients and their families while supporting care coordination, resource navigation, advocacy, and interdisciplinary collaboration.

This clinician will work closely with families navigating complex developmental diagnoses, emotional and behavioral challenges, and access to community supports. The ideal candidate is compassionate, organized, systems-oriented, and comfortable working within a fast-paced multidisciplinary pediatric environment.

ESSENTIAL RESPONSIBILITIES

Direct Clinical Services

  • Provide direct therapeutic services to children and families, including group, individual, family, and caregiver-focused interventions

  • Support clients experiencing emotional regulation challenges, anxiety, adjustment difficulties, behavioral concerns, trauma, and family stressors

  • Provide parent coaching, psychoeducation, and caregiver support related to child development, emotional wellness, communication, and family systems

  • Develop and implement individualized treatment plans aligned with client and family goals

  • Maintain timely and accurate clinical documentation in accordance with ethical, payer, and organizational standards

  • Participate in crisis support, safety planning, discharge planning, and continuity of care efforts as needed

Family Support & Case Management

  • Assist families in systems, insurance barriers, and community resources

  • Provide case management support and care coordination for families with complex support needs

  • Connect families with internal and external services and supports including respite care, financial resources, transportation services, and mental health providers, etc.

  • Identify barriers to treatment participation and collaborate with families and internal teams to problem-solve solutions

  • Support families through transitions in care and changes in service needs

Interdisciplinary Collaboration

  • Collaborate with multidisciplinary teams including ABA, speech, occupational therapy, physical therapy, academy, nursing, and leadership to support whole-child outcomes

  • Participate in interdisciplinary treatment planning and collaborative problem-solving

  • Provide behavioral health insight and family systems perspective to multidisciplinary care planning

  • Work side-by-side with leadership and clinical teams to strengthen family support systems and elevate integrated care practices across the organization

QUALIFICATIONS

Required Qualifications

  • Master’s degree in Social Work (MSW)

  • MUST BE LICENSED OR LICENSE ELIGIBLE IN THE STATE OF ARIZONA (LMSW, LCSW, or equivalent) AND BE IN GOOD STANDING

  • Experience working with children and families in pediatric behavioral health, developmental disabilities, education, healthcare, or related settings

  • Strong understanding of child development, family systems, trauma-informed care, and neurodiversity-affirming practices

  • Excellent communication, collaboration, and organizational skills

  • Ability to collaborate effectively within a multidisciplinary team

Preferred Qualifications

  • Experience supporting autistic children and/or children with developmental disabilities

  • Experience supporting families navigating complex systems

  • Pediatric mental health or integrated care experience

  • Experience collaborating with ABA, speech, occupational therapy, or educational teams

  • Bilingual candidates strongly encouraged to apply

PHYSICAL REQUIREMENTS

  • Must be able to pass a background check

  • Must obtain Fingerprint Clearance Card

  • Must acquire certifications in CPR / First Aid

  • Must be able to lift 50 lbs. and provide physically active engagement with children
